Who May Qualify

  • Age 18-70 years
  • Fibromyalgia diagnosis (ACR 2010/2016 criteria)
  • Symptoms present for at least 3 months
  • Average pain score ≥4 on 0-10 scale
  • Widespread Pain Index (WPI) ≥7 and Symptom Severity Score (SSS) ≥5
  • Stable medications for at least 4 weeks

Who May Not Qualify

  • Inflammatory arthritis or autoimmune disease
  • Untreated sleep apnea
  • Uncontrolled psychiatric disorder
  • Current opioid use >30 morphine mg equivalents daily
  • Pending disability or workers compensation claims
